OBJECTIVE: This article provides instructions on how to identify and flag clients in EvokeEHR who are participating in the HealthyMATCH program.
NOTE: Clients who are “on hold” due to being in the hospital, short-term rehab, etc. can remain active. Clients who have decided not to participate or have unenrolled for any reason should not have the HealthyMATCH toggle on in EvokeEHR.
1. Navigate to DataLink Software: Login and enter your Username and Password. Click Login.
2. Search for the client you want to mark as a HealthyMATCH client by clicking on the magnifying glass icon in the top-right corner of the screen.
3. Navigate to the client’s Face Sheet by hovering over the client’s initials on the left menu panel. Select Face Sheet from the drop down menu.
4. Scroll down through the Face Sheet to the HealthyMATCH buttons. Click Yes to mark that the client is part of the HealthyMATCH program. Click Save.
