Objective: This article provides a timesheet for Ohio Caregivers to be submitted if there are any issues with clocking in and out of a visit.
You will need a timesheet if you encounter any of the following:
- Unable to clock in and out due to technical issues
- No clock in and/or clock out times
- Visit has no tasks completed
- Visit has no client signature
- GPS location was not verified
